Second Paragraph - This contains the required citation under which
the request is made.
Third Paragraph - Indicate the method by
which you wish for the certificate of documentation to be returned. You can
also provide an account number for Federal Express, Airborne, and other
carriers, a prepaid label or accompany the request with by prepaid
packaging. Other delivery instructions will be honored when feasible.
Signature Requirements
All requests must contain the owner's signature and can not be signed by an agent. Signature notarization is not required.
