Official Number - Insert the official documentation number if already assigned.
Hull Number - Insert the hull number. This is mandatory if the official number has not yet been assigned.
Requester Names - Insert the name or names as shown on the Application for Documentation.
Planned Departure Date - Insert the date on which the vessel will embark into foreign waters if this is the case.
City of Departure - Insert the city and country from which the vessel will depart if this is the case.
City of Arrival - Insert the city and country for the vessel's destination.
First Paragraph - Insert the requester's name and reason for making the request.
Second Paragraph - This contains the required citation under which the request is made.
Third Paragraph - Indicate the method by which you wish for the certificate of documentation to be returned. You can also provide an account number for Federal Express, Airborne, and other carriers, a prepaid label or accompany the request with by prepaid packaging. Other delivery instructions will be honored when feasible.

Signature Requirements

All requests must contain the owner's signature and can not be signed by an agent. Signature notarization is not required.
