Hull Identification Number Application

Usage Context

In order to qualify for vessel documentation, the subject vessel must contain a properly formatted hull identification number that complies with federal regulations. Although typically assigned and installed by a manufacturer, this may not be the case for homemade or certain imported vessels. If the boat is intended for your own private usage, you may  request a hull number assignment directly from the U.S. Coast Guard. This will have a "USZ" designation in place of the usual Manufacturer's Code (MIC).

Citations and Instructions

Owner Information - Provide your name and address as it will be shown on the Certificate of Documentation. Include a telephone number where you can be reached should there be any questions. Indicate whether you are applying as the builder or whether it is being imported from another country. If you personally constructed the boat, it will be necessary to declare the number of other boats you built within the past 10 years. If such number is excessive, the Coast Guard may require that you apply for a formal Manufacturer's Identification Code (MIC). When the vessel was imported, you will need to insert the name of such party.
Vessel Information
- Include the required vessel information along with the construction finalization date and a state registration number if applicable.
Vessel Builder Information - Provide the requested information if the vessel was built by a party other than yourself.
Signature Requirements - The form should be signed by the owner or a designated agent. Notarization is not required.
